Before you start this intake form, we recommend that you have access to your medical records to help you answer some of the questions and will also help with your medical IMO’s and Nexus Letters if needed. Make sure you uploaded all documents to “Personal File” section of your Executive Member Portal to help support your claim .

Fill out all the information below to the best of your ability. If the Healthcare Provider may have any questions, he/she will contact you directly. The medical team will provide a medical opinion based on the documentation you have in your records. The doctors are 100% independent and cannot add items that are not in the records for which there is not an established diagnosis. We have a team working for you behind the scenes that helps facilitate the review with our medical team. The medical team consists of independent medical examiners. They do NOT work for VHV2020, Inc. We’ve done it this way to avoid any perceived or actual conflict of interest regarding your VA disability claim. Here are the fees you can expect AFTER you submit this form, which you pay directly to the provider working your case: For $200 you will receive an Independent Medical Opinion w/ Nexus Statement (per condition).
